Hospice Community Liaison
Position Summary:
The Hospice Community Liaison serves as both a compassionate advocate and a strategic sales professional for EverHeart Hospice. This role is responsible for developing and maintaining strong relationships with key referral sources, including assisted living facilities, skilled nursing facilities, hospitals, and physician offices. The Liaison actively promotes EverHeart’s hospice and palliative care services through targeted outreach, educational initiatives, and consistent relationship management to drive referral growth and expand access to care.
This is a hybrid role based out of our Coldwater, OH office, with regular travel during business hours throughout Mercer, Van Wert, Auglaize, Allen, and Paulding counties, and surrounding areas as needed. Evening and weekend event attendance may be required for community engagement and outreach activities . A company-provided fleet vehicle is available for all approved business travel.
Education and Experience:
- Associate’s degree in Marketing, Business, Healthcare Administration, Nursing, or related field preferred.
- Equivalent experience in healthcare, sales, or community outreach will also be considered.
- Minimum of 2 years of experience in healthcare, ideally in hospice, home health, or senior care preferred.
- Healthcare sales, marketing, or business development experience strongly preferred.
Additional Requirements:
- Valid driver’s license with an acceptable driving record; required for use of company-provided fleet vehicle.
- Strong interpersonal and customer service skills with the ability to build and maintain professional relationships.
- Excellent communication, documentation, and organizational skills; detail oriented.
- Self-directed with the ability to work independently and manage time efficiently.
- Willingness to travel regularly within the service area, including in inclement weather.
- Ability to lift up to 25 lbs. of materials or equipment as needed.
- Physically able to perform job duties, including sitting, standing, walking, bending, and lifting.
Travel Requirements:
- This is a hybrid position based at the EverHeart Hospice office in Coldwater, Ohio.
- The role involves regular travel within Mercer, Van Wert, Auglaize, Allen, and Paulding counties, and surrounding counties as needed.
- Travel occurs during regular business hours; however, evening or weekend attendance at community events, health fairs, or educational sessions may be required.
- A company-provided fleet vehicle is available for all business-related travel.
Position Responsibilities:
- Develop and maintain detailed profiles for referral sources, including key personnel, occupancy data, needs assessments, and referral trends to support targeted outreach.
- Conduct regular outreach to physicians and healthcare facilities (hospitals, skilled nursing, rehabilitation, assisted living, etc.) within EverHeart’s service area to generate new referrals and drive census growth.
- Coordinate and facilitate training and educational sessions for referral sources in collaboration with appropriate internal team members or subject matter experts.
- Represent EverHeart Hospice at community programs, health fairs, educational events, and other outreach initiatives to enhance visibility and engagement.
- Serve as a liaison between referral sources and EverHeart staff to communicate service offerings, resolve issues, and maintain positive, long-term relationships.
- Conduct sales calls, arrange appointments, and deliver targeted presentations to keep referral sources informed and engaged with EverHeart’s hospice and palliative care services.
- Build and sustain strong relationships with healthcare professionals and community stakeholders to support referral growth aligned with service area targets.
- Collaborate with the Director of Marketing to develop and implement a goal-driven outreach and marketing plan focused on increasing census within the defined service area.
- Meet regularly with internal staff to discuss referral activities, new opportunities, challenges, and provide insights to optimize census growth efforts.
- Work closely with the Business Development Team and Admissions Specialists to identify and resolve barriers affecting referrals and admissions.
- Utilize and analyze referral data through Census Dashboard and other tools to track progress toward census and service area goals.
- Monitor local market conditions, competitive activity, and emerging trends to inform strategic outreach and growth initiatives.
- Perform other duties as assigned to support EverHeart Hospice’s growth and community presence.
