- Estimated Weekly Pay: $2,192 - $2,436
- Mileage reimbursement provided
- Active RN license in Ohio
- Minimum 2 years of experience as a Home Health Registered Nurse
- Proficiency with OASIS and HCHB documentation systems
- Current CPR certification
- Valid driver's license, vehicle insurance, and reliable transportation
- Deliver skilled nursing care to patients in their homes throughout the Cleveland area
- Complete accurate and timely clinical documentation using OASIS and HCHB
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Educate patients and families on health management, post-acute care, and recovery
- Travel to patient homes within Cleveland, providing high-quality, patient-centered care
