PRN RN Community Hospice

Pay Range- $ 28.00- $47.54 Job Summary
We are seeking a compassionate and dedicated PRN RN-Community Hospice Nurse to provide high-quality care to patients in our hospice facilities. The ideal candidate will be responsible for delivering comprehensive nursing care, ensuring comfort, and supporting patients and their families during end-of-life transitions. This role requires strong clinical skills, empathy, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities

• Assess patient needs and develop individualized care plans based on their medical history and preferences.
• Administer medications and treatments as prescribed, including managing pain relief and symptom control.
• Monitor vital signs and perform basic life support as needed to ensure patient safety.
• Provide emotional support and education to patients and families regarding the hospice process and available resources.
• Coordinate with physicians, social workers, and other healthcare professionals to facilitate comprehensive care.
• Document patient progress and changes in condition accurately in medical records.
• Assist with airway management and other critical interventions when necessary.
• Participate in triage for urgent patient needs, ensuring timely responses to changing health conditions.
• Engage in home care visits, providing compassionate support in a familiar environment for patients.

Skills

• Proficient in nursing skills related to long-term care, including experience in nursing homes or similar settings.
• Knowledge of coding practices for accurate documentation of patient care activities.
• Ability to dialyze patients when required, demonstrating advanced clinical capabilities.
• Strong understanding of vital signs monitoring and basic life support techniques.
• Excellent communication skills for effective interaction with patients, families, and healthcare teams.
• Empathy and compassion for patients facing end-of-life challenges, fostering a supportive environment.
• Ability to manage complex patient situations with critical thinking and problem-solving skills.
