Job Summary:

Serves as a nutrition expert in the assessment and management of diabetes. Responsible for establishing and maintaining the diabetes self-management education (DSME) accreditation/recognition program through the American Diabetes Association and/or the American Association of Diabetes Educators. Develops, implements and evaluates evidence-based education, including diabetes self-management education and facilitates optimal health outcomes for patients with or at risk for diabetes. Provides evidence-based education for all patients with diabetes. Provides diabetes education to staff members and serves as a consultant to all healthcare providers. Develops and directs diabetes education programs to serve the diabetes community. Provides disease management education to patients in individual, group and community settings. Conducts and implements comprehensive nutritional assessments to a variety of outpatients at nutritional risk. Utilizes medical nutritional therapy intervention methods to resolve nutritional crisis and ensure health maintenance of patients. Collaborates and communicates with patient’s primary care physician. Serves as a resource to the community; develops, implements and evaluates outreach education programs and health screenings for the community. Assists to identify, develop, implement and evaluate disease management education programs and related resources for clinical staff members. Assists to develop evidence-based disease management protocols for patient care. Serves as a nutritional consultant for disease management to all healthcare providers. Demonstrates critical thinking to assure the delivery of safe, quality, age specific care. Delegates effectively to appropriate team members. Documents accurately and in a timely manner. Communicates and collaborates with other healthcare team members. Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.

PRIMARY JOB RESPONSIBILITIES


  • Develops, implements and maintains the diabetes self management education accreditation/recognition program through the American Diabetes Association and/or the American Association of Diabetes Educators.
  • Monitors, collects and analyzes diabetes disease management data including patient outcomes; implements/participates in related performance improvement initiatives.
  • Monitors, maintains and documents all required information/data for continued accreditation/recognition by the American Diabetes Association and/or the American Association of Diabetes Educators. Ensures and documents compliance of requirements of all healthcare team members involved in diabetes self management education.
  • Provides direction, tracks outcomes, and interfaces with the other members of the healthcare team to provide quality, comprehensive education to participants via the Living with Diabetes program.
  • Seeks traditional and non-traditional ways to enhance/grow the diabetes education self-management accreditation/recognition program.
  • Assumes initiative to educate self regarding specific disease management programs and provision of effective patient education.
  • Develops, implements and evaluates evidence-based disease management education programs for patients and the community.
  • Assesses/reassesses patients’ disease management educational needs based upon observation of patient and interview of patient and/or family, physician’s diagnosis and review of data, diagnostic results, and other relevant information from other healthcare providers.
  • Interviews patients, reviews patients’ medical and dietary history to determine food preferences and dietary habits. Assesses nutritional needs. Correlates information with patients’ medical status/progress and makes appropriate recommendations to physician.
  • Uses professional knowledge to develop/revise plans of care for patients according to evidence-based guidelines; establishes expected outcomes and evaluates/re-evaluates patient’s response to nutrition interventions and patient’s progress to expected outcome(s).
  • Develops and implements individualized dietary nutrition plans in accordance with the physician’s orders. Utilizes nutritional assessment methodology to include planning individualized menus, nutrient calculations and modification of diets.
  • Evaluates current pharmacologic therapies and uses advanced clinical skills to calculate nutritional needs; appropriate recommendations to physician.
  • Provides patients and family members (as applicable) with evidence-based health care education and disease management instructions. Integrates information about nutrition, physical activity and medication use in relation to blood glucose results; teaches critical thinking to patients in the management of their disease.
  • Conducts and implements comprehensive nutritional assessments to a variety of outpatients at nutritional risk. Utilize medical nutritional therapy intervention methods to resolve nutritional crisis and ensure health maintenance of patients.
  • Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care.
  • Documents all aspects of patient care/education and maintains patient records and care plans in a timely manner.
  • Responds to inpatient diabetes and outpatient disease management referrals in a timely manner.

Requirements Include

    • Bachelor’s Degree in Nutrition/Dietetics or related field required
    • Maintains current registration status with the Commission of Dietetic Registration
    • Current licensure to practice Dietetics in the State of Ohio
    • Certification as a Diabetes Educator required or completed within designated timeframe
    • Minimum of 3 years experience
    • Teaching experience in a patient care, classroom or community setting preferred
