Key Responsibilities:

• Deliver direct care and monitor resident health
• Administer medications and treatments
• Maintain accurate documentation using EMR systems
• Communicate with residents families and healthcare professionals

Qualifications:

• Valid LPN license in Pennsylvania
• Proficiency in electronic health records and medication software
• Strong communication and organizational skills
• Compassionate detail-oriented and adaptable
• Completion of Fellowship Orientation and pre-hire assessment
