Position Summary

Assign appropriate ED E/M level, Assigns accurate ICD-10-CM and CPT/HCPCS codes to outpatient diagnoses and procedures. Closely reviews each medical record in order to arrive at the most accurate and optimal Ambulatory Payment Classification (APC). Abstracts outpatient charts.

Responsibilities

  • Code all charts by assigning accurate codes (ICD-10, DRG, E, V, CPT/HCPCS) for health system billing and statistical purposes.
  • Obtain outpatient charts and other necessary medical record information in a timely manner in order to code accounts within a reasonable time frame.
  • Closely monitor status of un-coded accounts to maintain unbilled receivables at lowest possible levels.
  • Obtain appropriate documentation to support assigned codes.
  • Insure conformity to government-required coding policies and guidelines.
  • Identify trauma cases and enter into Trauma Registry.
  • Accurately enter abstracted information and codes into the computer.
  • Participate in cross-departmental efforts to ensure billing and coding is efficient and effective in processing information to ensure successful billing of patient accounts.

Requirements

  • Completion of an accredited two-year program in Medical Records Technology at a technical school or college.
  • CPC or CCS certification required
  • 2 years actual coding experience is recommended, preferably within an acute care hospital, in order to function at this level of proficiency.
  • Knowledge and familiarity with personal computers.
  • LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
