We are seeking a skilled and compassionate Physical Therapist (PT) to join our growing team. In this role, you will help patients improve mobility, regain independence, and enhance their quality of life—all within the comfort of their own homes.

Compensation & Benefits

Per-Visit Rates:

• Start of Care / Resumption: $100 per visit ($90 base + $10 travel)
• Routine Visits: $55 per visit ($45 base + $10 travel)

• Flexible schedule – choose a full caseload or PRN assignments based on your availability

Key Responsibilities

• Perform comprehensive physical therapy evaluations, including OASIS assessments
• Develop, implement, and update individualized treatment plans
• Deliver skilled, patient-focused therapy to improve mobility, safety, and independence
• Document care in compliance with Medicare and agency requirements
• Collaborate with RNs, PTAs, physicians, and other clinicians to ensure coordinated care
• Provide clinical oversight and supervision to PTAs as appropriate
• Educate patients and caregivers on therapeutic exercises, mobility techniques, and home safety

Qualifications

• Current Ohio Physical Therapist license (required)
• Previous home health experience preferred (SNF or outpatient experience will also be considered)
• Knowledge of OASIS and Medicare guidelines a plus
• Strong clinical judgment, communication, and organizational skills
• Valid driver’s license, reliable transportation, and active auto insurance

Schedule & Territory

• Schedule: Monday–Friday, flexible hours based on caseload
• Coverage Area: West Chester Township and surrounding communities
