Case Management Coordinator – OhioRISE (Northeast Ohio Region)
Location: Northeast Ohio (Field-based | Telework Position)
Counties Served: Cuyahoga, Summit, Marion, Sandusky, Stark, Belmont, Jefferson, Huron, Harrison, Lake, Geauga, Lorain
Schedule: Monday – Friday | 8:00 AM – 5:00 PM (Flexibility Required)
Payrate : 27.17 an hour

We are seeking a Case Management Coordinator to join the OhioRISE team in supporting children with complex behavioral health needs through coordinated care and community support. This is a full-time field-based telework position requiring up to 50% or more travel within Northeast Ohio. Candidates must reside in Ohio, preferably in Cuyahoga or northern Summit County .

Key Responsibilities:

· Community-Based Case Management: Serve children and families across child-serving systems including mental health, child welfare, juvenile justice, and developmental disabilities.

· Member Evaluation: Conduct comprehensive needs assessments using care management tools, identify risk factors, and recommend individualized care plans.

· Care Plan Coordination: Implement, monitor, and adjust care plans; collaborate with Child and Family Teams and providers.

· Quality & Compliance: Monitor outcomes, escalate quality of care issues, and document per regulatory and company standards.

· Engagement & Education: Use motivational interviewing to empower members, support lifestyle/behavior change, and enhance member engagement.

· Barrier Resolution: Work holistically with clinical and community teams to resolve access and care issues.

Requirements:

· Education: Bachelor’s degree or non-licensed master’s in behavioral health, human services, health services, or public health (e.g. Psychology, Social Work, Counseling).

· Experience:

o 2+ years in behavioral health, social or human services.

o 2+ years in children’s mental health, child welfare, juvenile justice, or related fields.

o Familiarity with Ohio’s public systems and local community resources.

o Experience in case management, family counseling, or child development.

· Technical Skills: Proficiency in MS Office Suite (Outlook, Word, Excel, SharePoint) and multi-system navigation.

· Travel: Reliable transportation required; mileage reimbursement provided. Must be able to travel within the region up to 50% of the time. Occasional travel to the New Albany office for training/meetings.

Preferred Qualifications:

· Case management or discharge planning experience

· Background in Managed Care or Medicaid

· Prior experience with OhioRISE or similar Medicaid programs
