Case Manager -Youth Engagement

Coleman Health Services, a nationally recognized not-for-profit mental health and substance use services provider, is seeking Youth Engagement Case Managers to join our team! This position works in various community settings throughout Logan and Champaign Counties, Ohio. Youth Engagement Case Managers at Coleman work collaboratively with Central Ohio Youth Center (CYOC) to provide services currently not available to youth and their families within that system. This is a Monday - Friday position with regular scheduled hours; employee must be willing to work after 5pm a few nights a week to best meet the needs of youth and families after school hours. Will be off on all nine paid company holidays and no on-call requirements.

Job Duties of Youth Engagement Case Managers Include:

• Provide intensive, community based, and comprehensive case management services to clients and their families.
• Assist in identifying resources, supports and services based on youth and family needs.
• Work collaboratively with treatment teams internally and externally to achieve treatment plan goals.

Qualifications of Youth Engagement Case Managers include:

• Bachelor's degree in a social service-related field OR eligible as a Qualified Behavioral Health Specialist with at least one year experience.
• Willing to get NPI and Medicaid Billing number (we'll help!)
• Maintain excellent driving record
