We are seeking a motivated and caring Physical Therapist (PT) to join our clinical team. This position offers the opportunity to help patients restore mobility, regain independence, and enhance their quality of life —all from the comfort of their own homes.

Compensation & Benefits

• Per-Visit Rates:
• Start of Care / Resumption: $100 per completed visit (includes base rate and travel reimbursement)
• Routine Visits: $55 per completed visit (includes base rate and travel reimbursement)
• Flexible Scheduling: Choose between a full caseload or PRN assignments based on your availability

Key Responsibilities

• Conduct comprehensive physical therapy evaluations , including OASIS assessments
• Design and implement individualized treatment plans in collaboration with the care team
• Deliver skilled, patient-focused therapy to enhance mobility, balance, and safety
• Document all care accurately and promptly in compliance with Medicare and agency standards
• Coordinate care with RNs, PTAs, physicians, and other healthcare professionals
• Provide supervision and clinical oversight to PTAs as required
• Educate patients and caregivers on therapeutic exercises, mobility techniques, and home safety

Qualifications

• Active Ohio Physical Therapist (PT) license (required)
• Home health experience preferred (SNF or outpatient backgrounds also welcome)
• Familiarity with OASIS and Medicare documentation preferred
• Excellent communication, time management, and clinical skills
• Valid driver’s license, reliable transportation, and proof of auto insurance

Schedule & Service Area

• Schedule: Monday–Friday, flexible hours based on caseload and availability
• Coverage Area: West Chester Township and nearby communities
