Health and Wellness Coach

Summary

The position is responsible for the delivery of Whole Health Coaching and skill development of the Cincinnati VA Medical Center's Whole Health Coach program - including provision of care through coaching - promotion - education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey - specifically through the Pathway - wellbeing and Whole Health clinical care elements of Whole Health care.

Duties

• The Cincinnati VA Medical Center is a two-division campus located in Cincinnati - Ohio and Fort Thomas - Kentucky serving 15 counties in Southwest Ohio - Northern Kentucky - and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue - KY
• Florence - KY
• Lawrenceburg - IN
• Hamilton - OH
• Clermont County - OH - and Georgetown - OH
• Cincinnati VAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division
• Duties Include: Contact eligible participants for enrollment and explain the health coaching process
• Provides health coaching for Veterans in order to reduce or eliminate high risk behaviors while increasing healthy ones
• Conducts interventions for Veterans using one on one - couple or group modalities in outpatient and inpatient settings
• Conducts Whole Health facilitated groups' participate in MOVE - smoking cessation - diabetes management - hypertension clinics - mental health and other inpatient and outpatient groups Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments
• Coordinates care with Veterans' treatment providers - both primary and specialty care - to ensure identified goals are consistent with best practices for treatment and management of chronic illness
• Reviews and develops treatment plans for/with Veterans
• Serves on healthcare system - VISN - and/or national VA committees or task forces specifically relating to Whole Health or Health Coaching
• Assists with data management analyses - special studies and reports for use by management for budget preparation - workload projections - and quality assurance and utilization reviews
• Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues
• Conducts outreach activities to inform Veterans and family members of available programs Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory - qualification - and performance requirements are met
• Selection at a lower grade level does not guarantee promotion to the full performance level
• Work Schedule: Monday-Friday 8:00am-4:30pm Telework: Not Eligible Position Description/PD#: Health and Wellness Coach/PD99325-S Permanent Change of Station (PCS): Not Authorized Physical Requirements: While some of the work is sedentary including researching - writing - program planning and data analysis - the position will have periods of extensive walking and movement throughout the healthcare system - prolonged standing - and transporting light to medium weight objects
• The Whole Health Coach will also be required to travel between sites (CBOCs and outlying clinics) encompassed in the healthcare system
• The work is conducted in a medical center environment and involves risk of exposure to a variety of contagious diseases
• The work area is adequately lighted - heated - air conditioned - and ventilated.
