Duties and Responsibilities
- Conduct comprehensive patient assessments to diagnose behavioral and psychiatric conditions.
- Initiate and maintain effective treatment plans together with healthcare team members.
- Provide medication management and education for patients, ensuring adherence to therapeutic standards.
- Regular home visits to monitor patient progress, adjusting care plans as necessary.
- Documentation of patient care in compliance with legal, ethical, and institutional standards.
- Collaborate with family members and other caregivers to educate and inform on the patient's condition and care needs.
- Respond to crisis situations and stabilize patients using therapeutic communication and intervention techniques.
- Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
- Participate in continuous professional development to enhance skills and knowledge in behavioral health care.
- Advocate for patients to ensure access to necessary health care resources.
Requirements
- Valid RN license in the state of practice.
- Minimum of 2 years’ experience in behavioral health or mental health settings.
- Recent experience in settings like acute psychiatric units, outpatient services, or psychiatric home care is preferred
- Experience with Dementia/Alzheimer Patients
- Experience with home health care services is highly desirable.
- Advanced knowledge of psychiatric nursing principles and protocols.
- Ability to work independently and manage a caseload effectively.
- Strong interpersonal and communication skills, capable of effectively managing sensitive situations.
- Familiarity with electronic health record (EHR) systems and proficient in clinical documentation.
- Current CPR certification.
- Valid driver’s license and dependable transportation for home visits.
- A strong commitment to patient safety, privacy, and dignity.
- Flexible schedule, including availability for evening or weekend visits if required.
