DUTIES AND RESPONSIBILITIES

1. Supports and demonstrates practices resident care consistent with the mission, philosophy, standards, policies, and procedures of the department.
2. Delegate assistance with care to nursing assistants, providing direction and supervision of the tasks...
3. Create and maintain an atmosphere of personal interest and a positive emphasis on a calm environment throughout the neighborhood.
4. Demonstrate that the community is home for our residents.
5. Actively participate in care conferences and others as required.
6. Assist nursing assistant staff in providing direct care when needed.
7. Actively assist nursing assistants with answering resident initiated call light service requests.
8. Provide leadership with coaching and mentoring to the neighborhood team.
9. Support seamless service between shifts with good communication.
10. Develop and maintain good working relationship with personnel in other departments within the facility to assure that resident care services and activities can be adequately maintained to meet the needs of the residents.
11. Assures resident care staff complies with facility’s policies and procedures.
12. Assures that practices are consistent with regulatory and state requirements.
13. Identifies problems and makes recommendations for addressing these problems.
14. Initiate and retain the residents’ clinical record in accordance with facility policies and state requirements.
15. Make informative entries in the progress notes that reflect the care provided, and the resident’s response to that care.
16. Keep Nursing Supervisor informed of the status of residents and other related matters thru oral written reports. Assist Administrator with any duties assigned. Take call as needed.
17. Complete medication passes in accordance with state regulation and facility policy.
18. Communicates problems and needs to other departments.
19. Assists with managing, documenting, and investigating incidents and accidents.
20. Assists and participates in activities related to facility inspection or investigation by regulatory agencies.
21. Consult with attending physician as needed for treatments, medications, rehabilitation, etc. and coordinate such treatments and rehabilitation with 3 rd party agencies.
22. Notify attending physician, family and other staff when there is a change in the resident’s condition.
23. Manage resident emergencies, evaluation of resident conditions and completion of Incident Report.
24. Adheres to professional nursing codes of ethics.

SPECIFIC REQUIREMENTS

• Maintain current LPN License in the state.
