MAIN FUNCTION:

Plans, organizes and directs medically approved OT programs for treatment of musclo-skeletal defects, neuromuscular diseases and problems to restore function, relieve pain and prevent disability, taking into consideration patient's age, mental status and educational level. Responsible for supervision of OTAs.

RESPONSIBLE TO: Manager, Home Health

MUST HAVE REQUIREMENTS:

Bachelor of Science Degree in OT from an accredited college or a university.

Licensed by the State of Ohio through the OT, PT and Athletic Trainers Board.

BLS prior to independent patient care.

Operates a mechanically sound vehicle, maintains Ohio Drivers License and proof of auto insurance.

No written disciplinary action within the last 12 months.

PREFERRED ATTRIBUTES:

1 year experience in acute care, rehab center, SNF or Home Health.

Good decision making skills and able to function independently.

Good communication and effective time management skills.

Registered as an OT by AOTA.

OTHER REQUIREMENTS:

Employee in this position may be:

- exposed to blood, body fluids and/or airborne pathogens and is required to wear barriers.

- exposed to inclement weather.

POSITION EXPECTATIONS:

* Performs occupational therapist treatment and evaluation as prescribed by the physician, evaluating and treating clients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test.

* Utilizes interview techniques, observations, formal and informal screening/ evaluation tools to assess overall level of patient. Identifies patient's strengths and weaknesses.

* Collaborates with patients in goal setting, including family as appropriate.

* Completes an objective initial evaluation within 48 hours of referral. Documenting age appropriate, objective goals and treatment plans.

* Re-evaluation and discharge completed in a timely manner with completed documentation.

* Visits completed according to plan of care; visit frequency and choice of treatment activities.

* Assesses clients appropriately for fall risk interventions.

* Maintains continuity of care between patient and therapist schedules.

* Informs physician promptly and accurately of pertinent changes in patient condition.

* Contributes to staff meetings as needed.

* Collaborates and interacts appropriately with team members, including medical staff, insurance carriers, as well as fellow staff members.

* Instructs clients, family members and other health team members to incorporate therapeutic and self care activities into the treatment program, encouraging them so they can safely perform or supervise the activities.

* Assumes authority of COTA/L and supervises field competency, performing supervisory visits every month.

* Acts as a role model to staff, students and volunteers within the framework of accepted standards.

* Maintains all documentation clearly, according to standards and within required time frames.

* Completes OT quarterly clinical chart audits in a timely manner.

Actively participates in Home Health Department committees, staff meetings, and in-service programs as required.

Participates in at least one continuing education course each year.

Performs other tasks as requested.
