Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Outpatient clinic experience required. Candidate must have experience conducting 60-minute evaluations and treatment sessions.
• No new graduates. Minimum recent clinical experience required. (Preferred: demonstrated proficiency with outpatient musculoskeletal evaluation and plan of care development.)
• Ability to complete clear, concise, and timely documentation including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry per clinic and payer guidelines.
• Willingness to participate in student supervision and precepting as assigned.
• Maintain professional licensure and meet state-mandated continuing education requirements. Adhere to discipline-specific State Practice Act and regulatory standards.

• Facility Type: Outpatient clinic.
• Duration: 13 week assignment.
• Shift Pattern: 5x8 schedule.
• Position Type: Physical Therapist responsible for 60 minute evaluation plus treatment sessions.
• Clinical Focus: Outpatient musculoskeletal and functional rehabilitation with emphasis on return to community activities.
• Responsibilities include evaluation, treatment plan development and modification, delegation and direction of discipline-specific therapy services for assigned patients, and adherence to safety and compliance policies.
