JOB SUMMARY
Enhanced Community Care Management (ECCM) is seeking a dedicated Geriatric Clinical Pharmacist to join our expanding team, focusing on optimizing medication regimens and enhancing quality of life for complex geriatric and palliative care patients. This role emphasizes proactive deprescribing strategies and patient-aligned goals to reduce polypharmacy and burden, particularly at clinical inflection points. The successful candidate will be an embedded team member, leveraging strong interdisciplinary collaboration skills to actively influence providers and implement clinical interventions that significantly improve patient outcomes and contribute to ECCM's commitment to goal-concordant care.
ESSENTIAL RESPONSIBILITIES

Review gaps in care for high risk patients ?ss part of a multi-disciplinary team.Conduct research including gathering claims data, reviewing clinical websites, etc. to thoroughly evaluate each patient's unique situation to provide the appropriate clinical recommendation.

Contact patients, care givers, physicians, pharmacists, etc. to intervene on identified gaps of care using a high level of clinical judgement and discretion.

Pay Range Minimum:
$118,400.00
Pay Range Maximum:
$196,800.00
Base pay is determined by a variety of factors including a candidate's qualifications, experience, and expected contributions, as well as internal peer equity, market, and business considerations. The displayed salary range does not reflect any geographic differential Highmark may apply for certain locations based upon comparative markets.
