Physical Therapist – Full-Time (SNF/LTC)

Join our team as a full-time Physical Therapist in a split position between Phoenix of Fairlawn and Phoenix of Maple Heights. This role offers the opportunity to work in skilled nursing and long-term care settings, providing meaningful care to residents while being part of a supportive and collaborative team.

Position Overview
As a Physical Therapist, you will provide skilled therapy services to residents in a SNF/LTC setting. You will evaluate patients, develop individualized treatment plans, and help residents improve mobility, function, and overall quality of life. This split role offers variety and collaboration across both locations.
Qualifications
Licensed or eligible for licensure as a Physical Therapist
Interest in working with geriatric and medically complex populations
Strong communication and teamwork skills
Ability to work independently in a fast-paced environment
New graduates are encouraged to apply
