Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience required.
• Experience with Homecare Homebase (HCHB) electronic medical record required.
• Ability to manage a weekly patient caseload consistent with home health expectations (client notes indicate 32 visits weekly).
• Reliable transportation to travel within a defined territory covering Guernsey County and eastern Muskingum County.
• Valid driver’s license and required insurances for home health travel.
• Preferred: prior travel PT experience and familiarity with territory routing in rural or semi-rural settings.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday–Friday, 8:00 AM–5:00 PM.
• Typical weekly schedule: Monday–Friday, 40 hours per week.
• Territory: Guernsey County and eastern Muskingum County. Travel within this territory is required.
• Positions Available: 1.
