Hospice Registered Nurse 8 Week Contract
Job Summary
Join our dynamic healthcare team as a Hospice Registered Nurse on an 8-week contract, where you will play a vital role in providing compassionate end-of-life care to patients and their families. Requirements
Must have at least 1 year of Hospice experience within the last 3 years as an RN.
HCHB charting experience is also required.
Responsibilities
- Conduct comprehensive patient assessments to determine eligibility and develop personalized care plans aligned with hospice standards.
- Administer medications and treatments with precision, ensuring symptom management and patient comfort.
- Collaborate with interdisciplinary teams including physicians, social workers, and chaplains to coordinate holistic care approaches.
- Educate patients and families on disease progression, symptom control, and available support resources.
- Monitor patient vital signs, evaluate physiological changes, and adjust care strategies accordingly to optimize comfort.
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations for confidentiality.
- Facilitate discharge planning and coordinate post-acute or outpatient services as needed to ensure seamless transitions of care.
Requirements
- Valid nursing license with current registration in the state of practice.
- Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers is highly desirable.
- Strong background in hospital medicine, medical-surgical units, or post-acute care environments.
- Knowledge of physiology, diagnostic evaluation procedures, and medical terminology essential for accurate patient assessment.
- Experience with infectious disease care, behavioral health, geriatrics, pediatrics (PICU), or outpatient clinics is a plus.
- Familiarity with HIPAA compliance, discharge planning processes, and medical records management.
- Excellent communication skills with the ability to provide empathetic patient service and family support.
- Ability to work independently while collaborating effectively within multidisciplinary teams in fast-paced settings.
