Admissions Consultant - Palliative Care

Summary of Position: The Admissions Consultant is responsible for providing administrative support services to the admissions team, including all Agency programs; Hospice, In Touch and Palliative Care.

Education and Experience:

• High school diploma.
• Associate degree in Medical Records, Medical Assisting, or equivalent preferred.
• Minimum of one year of experience in healthcare setting.
• Hospice and Palliative Care experience preferred.
• Proficiency in using Electronic Medical Records system (EMRs) and Microsoft Office.

• Shall have a valid driver’s license.

• Detail-oriented with excellent communication, documentation, and organizational skills.
• Compassionate, professional, and respectful demeanor in sensitive care environments.
• Ability to follow instructions.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to lift 25 lbs. of supplies or equipment.
• Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.

Responsibilities of Position:

• Provide administrative assistance to the Medical Providers for the Palliative Care Program.
• Assist in patient intake, including obtaining medical history, medical records, medication records, and documenting health-related data.
• In coordination with patients and families, prepare and complete pre-admission paperwork according to Agency requirements.
• Provide consultative referral providing information on Palliative Care and end of life service to patients and families aimed at matching prospect with the best service from our portfolio.
• Scheduling appointments, managing patient flow, calling patients, communicating visit dates and times, calendar coordination, and ensuring timely care of Palliative Care patients.
• Communicating with Providers to assist with the coordination of care.
• Coordinate with the EverHeart interdisciplinary team for other patient needs.
• Track Palliative Care referrals and referral information. Coordinate all Palliative Care referrals.
• Participate and assist with the In Touch Program.
• Maintaining accurate and organized patient records, including electronic medical records (EMRs).
• Coordinate with the Admissions Team to determine the appropriateness and urgency of Hospice and Palliative Care referrals.
• Track the data including key performance indicators for Palliative Care Program and In Touch Program.
• Complete audits of Palliative Care and Hospice information for accuracy and related to quality and compliance standards.
• Provide phone coverage at other offices as needed.
• Serve as backup to the other admission consultant and medical secretaries including office opening and closing procedures.
• General clerical duties to include but limited to utilization of Microsoft Office products, photo copying, faxing, mailing, and filing.
• Direct incoming calls, greet visitors, route telephone and faxed messages appropriately as applicable to provide backup to the medical secretary.
• Other duties as assigned.
