Position Overview
We are seeking an experienced and motivated Director of Nursing (DON) to lead and manage our nursing services. The Director of Nursing will oversee clinical operations, ensure compliance with state and federal regulations, supervise nursing staff, and maintain high standards of patient care.

Key Responsibilities

• 
  Provide leadership and oversight to the nursing department
• Lead day-to-day clinical operations in compliance with Medicare, and Ohio State home health regulations
• Supervise, mentor, and evaluate nursing staff including RNs, LPNs and Therapists
• 
  Ensure timely and accurate documentation in Axxess EMR
• Manage QAPI program, clinical audits, and survey readiness
• Develop and implement nursing policies and procedures
• Conduct patient assessments and oversee care planning
• Coordinate with interdisciplinary teams to ensure quality patient outcomes
• Assist with staff training, onboarding, and clinical competency development
• 
  Maintain accurate documentation and regulatory compliance
• Experience with CHAP or ACHC accreditation

Qualifications

• Active Registered Nurse (RN) license in the State of Ohio
• Bachelor’s Degree in Nursing (BSN) preferred
• Minimum 3–5 years of nursing leadership or supervisory experience
• Experience in home health, community-based care, or long-term care preferred
• Strong leadership, organizational, and communication skills
• Knowledge of Medicaid waiver programs and home health regulations is a plus
