Medical Transporter

Provide transportation to and attend all client appointments (including dental, specialty, vision, psychiatric, and preventative care) and follow-ups for each client in the assigned cluster. Become familiar with and thoroughly knowledgeable of each client's ISP. Ensure all pre-appointment requirements, including paperwork, signatures, and notifications are ready and in possession at the appointment. Communicate and document all post-appointment work, including any follow-up needs/concerns, meds, and MAR updates with the MCC and Site Supervisor. Provide support and assistance to the client during the appointment, including preparing to discuss all relevant client medical issues with physician during the appointment and having all proper paperwork completed and accurate. Maintain positive relationship with all physicians and their staff.

Requirements:

Age 21 or older

United States Citizen or resident alien

Ability to lift 50 pounds and safely lift, turn, position and transfer individuals with physical disabilities.

Must meet the job and skill requirements of a Direct Support Professional (DSP).

Must have strong communication skills, both oral and written.

Must have a desire to serve individuals with developmental disabilities, and to create and maintain a caring environment for them.

Must have a belief in and understanding of agency's mission and ability to promote and communicate the philosophy, mission, and values to internal and external stakeholders.

Must have reliable transportation, a valid Ohio driver's license with fewer than three violations and no instances of OVI, vehicular homicide, suspension of license, and speeding 10+ mph over speed limit* in a 3-year period, and auto insurance.

Must have satisfactory health, background check.

Must successfully complete training and requirements to be a certified Med Passer within 90 days of hire.

Must complete training and documents to be an operator of agency vehiclesincluding vans and wheelchair lifts.
